Oxygen Transmissibility of Soft Contact Lenses Made Visible
Aside from topography and topographic keratoconus screening, the Keratograph 5M provides a large contact lens data base and many analyses for daily practice. The built-in keratometer and automatic measurement ensure the utmost accuracy and reproducibility. After completing the measurement, the overview display provides a detailed outline.
The refractive power of the front surface of the cornea consists of different components. The Fourier Analysis identifies four of them which are shown in the following displays:

  • Spherical component
  • Decentration
  • Regular astigmatism
  • Irregularities

Pathological changes can be quantified and possible effects on visual acuity can be explained.

Zernike polynomials are adapted to the elevation data of the cornea, which is crucial for locating the apex. The apex position is labelled with a cross. This display shows you if a back toric contact lens is applicable to the particular case. Zernike polynomials and the aberration coefficient give you important indications of the imaging quality of the corneal surface. Abnormal values are marked in colour.
Keratoconus classification is based on numerous parameters. The Topographic Keratoconus Screening display merges these parameters. The coloured label illustrates abnormal values. Temporal changes of the parameters are shown side by side in a table, to facilitate your follow-ups. The Amsler classification system is applied to the keratoconus domains.
Follow-ups require comparison of several examinations. In doing so, changes can be easily detected and fully documented. Regular follow-up examinations provide reliability and increase the trusting relationship between you and your patient. The Keratograph software contains both data and image documentation.

Comparing Examinations
With the Compare 4 Exams display you can now compare up to four examinations. Changes from the first to the latest measurement can easily be displayed, reflecting the course of disease over time. Select the examination that you wish to compare (A, B, C, D) with only two clicks and see the results right away – independently of the curvature type. The easy-to-understand display helps you describe even complex matters to your patient.

The resulting classification from corneal staining requires well-trained examiners. It is difficult to estimate the number of hyper-fluorescent dots on the corneal surface, but the integrated JENVIS Grading Scales facilitates this evaluation. Every image taken can be compared with a sample image on the screen. Vessel injections can also be evaluated and documented in this way.

Using the “Pupillometry” option is a quick and easy way to measure the pupil size of your patients under different illumination conditions. This option not only supports you when fitting multifocal lenses, but also when measuring the optical zone before refractive or cataract surgery.

The non-invasive Keratograph break-up time (NIKBUT) measures tear film stability. The NIKBUT is automatically measured within seconds, without fluorescein application. Human eyes are not able to perceive infrared illumination. Glare and reflex tear secretion are therefore avoided during the examination. This visualizes the results in an easy and undestandable way for you and your patients.
The Tear Map shows the affected areas: The respective break-up time is graphically illustrated for each Segment in seconds and according to the principle of a traffic light.
Evaluation of Lipid Layer
Hyper-evaporative dry eye is easily overlooked when using conventional tests. Thus evaluating the lipid layer of the tear film is even more important. With the Keratograph 5M you can record videos of interference patterns of the lipid layer. Distribution characteristics, morphology and thickness of the lipid film can be continuously evaluated.
Tear Film Dynamics
The tear film contains numerous particles. These can be made visible using a specific light source. These particles are distributed in the tear fluid from Bottom to top during each blink. The velocity of these particles provides information on tear film viscosity. You can quickly and easily evaluate the quantity and movement of these tear film particles using the TF-Scan.
With the Keratograph 5M, even difficult examinations such as meibography can be carried out easily. It visualizes meibomian gland dysfunction (MGD), the most common cause of dry eye disease. Morphological changes in the glandular tissue of the upper and lower eyelid are shown. Up to four images from examinations of the same eyelid can be compared in a single display to assess the patient’s progress.

JENVIS Grading Scales
These four-point grading scales allow easy classification of MGD based on an individual meibography. Reference-state images assist in classifying the gland structure as normal or indicative of mild or severe MGD.

Previously, conjunctival redness evaluation was subjective and varied depending on the examiner‘s qualifications. With the R-Scan it is possible to objectively classify bulbar and limbal redness fully automatically. The R-Scan detects vessels in the conjunctiva and evaluates the degree of redness. The automatic classification saves you laborious comparison and gives you more certainty in your asessment.

Bulbar and Limbal Redness
Different display options help to classify the degree of redness. Choose between camera image, view of fine vessels in the conjunctiva, red-free or contrast-enhanced display. Bulbar and limbal redness are evaluated in the temporal and nasal areas and all results are saved automatically.

An intact tear film and a good supply of oxygen to the cornea are an absolute necessity for wearing contact lenses comfortably. The OxiMap® presents a colour map of the oxygen transmissibility of soft contact lenses based on the lens power – which even your customers will understand!

Easy-to-Understand Patient Education
The oxygen transmissibility (Dk/t) depends on the material the contact lens is made of and on its thickness. In the past, only the manufacturers’ specifications about the oxygen transmissibility in the center of a -3,00 D contact lens were available. With the OxiMap®, you now get a graphic visualization of the Dk/t values over the whole area, based on the lens thickness.

The Dk/t values are colour-coded, whereby black represents an oxygen supply lower than that present in a closed eye. To preserve the integrity of the cornea when wearing contact lenses, minimum Dk/t values are recommended based on the length of time the contact lenses are to be worn. The OxiMap® colour-coding is based on these international recommendations.

Measurements With Placido Ring Illumination

White ring illumination is used to measure thousands of points on the corneal surface. Infrared ring illumination is also available for analyzing the tear film in order to prevent reflex tear secretion caused by glare.

FAQ

The NIKBUT measurement for dry eye with severe corneal epithelial damage is difficult. Even when the NIKBUT measurement is performed in such cases, the device mostly reports the results as “too short”.
The differences between fluorescein TFBUT and NIKBUT values should be noted, although positive correlation between TMH and Schirmer test values has been reported.
White areas are displayed if the patient blinks before 15 seconds after the measurement started. This does not affect the measurement in any way.
No, the software detects a basic image. In the case of keratoconus for example the strained Placido rings induced by the condition is noted initially by software and distortions from the original image is recorded and indicated as a break in the tear film. However, in cases with advanced keratoconus or severe irregular corneal astigmatism, it is hard to perform the measurement.
No, the area being analyzed during the NIKBUT measurement usually doesn’t coincide with the area affected by the nose shadow.
At least three measurement points (nasal and temporal limbus, pupil center) should be selected. To get the right value the ruler has to be placed on the lid margin and measure perpendicular up to the last reflex of the meniscus. Please see image as reference. The correct measurment is 0.22 mm for this example.

No, however grading scales called the JENVIS Grading Scales for meibomian glands have been included in the software. A sliding bar tool allows for direct comparison of patient results to the grading scale, making meibo grading very easy with the Keratograph 5M.
In eyes with small lid aperture or eyes with ptosis, (commonly seen in the older Asian population), the device cannot detect the conjunctival area correctly.
The particles are a combination of eroding corneal epithelial cells, tarsal conjunctival cells and small air-particles.
The purpose of the JENVIS Pro Dry Eye Report is to provide the clinician with various dry eye related examinations, improving workflow, aiding in the diagnosis and educating the patient. The clinician uses the results from the JENVIS Pro Dry Eye Report to formulate the diagnosis. Treatment options are included in the software, however these are chosen at the discretion from the clinician. Different treatment suggestions can be manually added.
No. The report is set up to allow clinicians to individualize the screening procedure if they so require. A screening option, consisting only 4 procedures, is also included. This is a good tool to screen every patient entering the clinic. If a problem is detected, more detailed examination with an individualized report can follow.
It is important to start from least invasive followed by more invasive tests. The workflow in the JENVIS Pro Dry Eye Report is set up to accommodate this principle.
Yes, data from the NIKBUT, R-Scan and all keratometric data can be exported to csv files.
Yes, with the 22 Placido disc 22.000 real measured data points are generated after each measurement. None of these are extrapolated measurements.

No, a keratometer is included in the hardware measuring real Ks of the anterior surface.

No, the Keratograph is a Placido topographer. Placido discs are projected on the tear film, the software analyzes the resulting images and measurement data of the anterior cornea is generated. A Pacido topographer can’t measure the posterior cornea on its own.

Topographic indices for keratoconus screening are incorporated in the software, as well as Fourier Analysis. Objective inspection of the sagittal map is another tool to aid with diagnosis.
No, lenses cannot be individually measured. Well-known soft lenses were analyzed and the information is programmed into the software. The role of the OxiMap® is to educate patient’s on the importance of using lenses with high oxygen transmissibility, showing them the benefits in an easy to understand image. Furthermore the OxiMap® gives information about the oxygen transmissibility over the entire surface of the lens, as well as differences between prescriptions.
